Supporting residents to live full and happy lives
Falkirk Health and Social Care Partnership is one of Scotland's 31 integration organisations, supporting approximately 160,000 people in rural and urban areas. Its goal is to help Falkirk residents live full and happy lives in supportive and inclusive communities.
Falkirk Council, NHS Forth Valley, and partners in the third and independent sectors employ the Partnership's staff. They work at Falkirk's local social work offices, health centres, GP practises, community hospitals, care homes, intermediate care facilities, day centres, and communities.
Falkirk Health and Social Treatment Partnership provide care and assistance to adults and older persons who are suffering from mental health issues, long-term physical ailments, or drug abuse issues.
Social care services and support choices are integrated and seamless
Carers who offer unpaid care for their loved ones are also supported by The Partnership through the Falkirk and Clackmannanshire Carers Centre. Falkirk Council, NHS Forth Valley, the third sector, service users and caregivers comprise the Partnership's Integration Joint Board.
The Board is responsible for planning, funding, and running services in order to meet the nine National Health and Wellbeing Outcomes, with an annual budget of about £255 million.
What is Health and Social Care Integration
Integration is the most important reform to Scotland's health and social care services since the NHS was established in 1948.
It guarantees that health and social care services and support choices are integrated and seamless, which is particularly important for persons with long-term or complicated needs.
Falkirk Community Hospital is working on a new master plan that will outline how present services may be enhanced, upgraded, and expanded across the campus.
Local workers, GP practises, NHS Forth Valley, Falkirk Health and Social Care Partnership personnel, and Falkirk Council are all working together on this.
