Within Glasgow City, the vision for community health and social care services is to support people to flourish, work in partnership with people and organisations to transform services for the better, and engage with communities to improve health and wellbeing.
Their meduim to long-term vision is:
"Communities will be empowered to support people to flourish and live healthier, more fulfilled lives, by having access to the right support, in the right place and at the right time."
Local Authorities and Health Boards are now required to plan and deliver community health and social care services together. This new way of working is referred to as ‘Health and Social Care Integration.’
Glasgow City Council and NHS Greater Glasgow and Clyde are jointly planning and delivering all of Glasgow’s community health and social care services – for children, adults and older people, including homelessness and criminal justice services. This is being done as the ‘Glasgow City Health and Social Care Partnership’ which is directed by the Glasgow City Integration Joint Board.

Health and Social Care Integration
The Glasgow City Integration Scheme outlines the collaborative approach between local authorities and health boards for planning, delivering, and monitoring health and social care services. It was approved in 2016, with updates in 2018 to align with legislative changes. The scheme establishes the Integration Joint Board, responsible for overseeing health and social care services in Glasgow City. Prior to its approval, various stakeholders were consulted to inform its development, as mandated by the Public Bodies (Joint Working) (Scotland) Act 2014.
Health and Social Care Integration aim to:
- Improve the quality and consistency of services for patients, service users, carers and their families
- Provide joined-up quality services where people are cared for in their own homes or in a homely setting where it is safe to do so and
- Ensure resources are used effectively and efficiently to deliver services that meet the increasing number of people with long-term and often complex needs, many of whom are older

Strategic Plan
The Glasgow City Integration Joint Board's Strategic Plan for 2023-26 outlines the overarching vision and priorities for health and social care services across the city. It's a comprehensive document developed through extensive engagement with various stakeholders, including patients, service users, carers, staff, and community partners. The plan details how integrated health and social care services are planned, delivered, and monitored within Glasgow. It highlights collaboration between Glasgow City Council, NHS Greater Glasgow and Clyde, and partners from the third and independent sectors. The plan is reviewed every three years, with input from the community to ensure that it remains relevant and responsive to evolving needs. It serves as a roadmap for improving health and social care outcomes for residents of Glasgow, guiding decision-making, resource allocation, and service delivery.
